Health Basics includes a Limited Medical Plan for Hourly, Management and Owners. The plan come with pricing for a voluntary offering, or pricing for a minimum of 50% Employer-Paid premiums. The Plan needs a minimum of 5 insured persons and is guaranteed issue.
